Italy is probably the cheapest genuine medical degree open to a Bangladeshi student, and the reason has nothing to do with scholarships. An Italian public university does not price medicine separately. It sets tuition for every degree it awards on the same income assessment it uses for law or economics: EUR 0-4,000 a year.
There is a price for that, and it is not paid in money but at admission: the hard part of medicine in Italy is the IMAT, not the bill. That belongs at the top, because most Bangladeshi families budget for medicine around what a private medical college charges, then choose a destination costing several times more on a number that does not apply here.

No, Italy did not scrap the entrance test — not for you
You will read everywhere that Italy has abolished its medicine entrance exam. The reform is real and it does not apply to the course you are applying to. Law 14 March 2025 no. 26, implemented by Legislative Decree 15 May 2025 no. 71, replaced the national entrance test for single-cycle Medicine and Surgery, Dentistry and Veterinary Medicine with an open first semester — the semestre filtro — in which places are decided by the end-of-semester exams. That reform covers Italian-taught medicine.
The English-taught programmes a Bangladeshi student actually applies to still admit through the IMAT, which ran again on 29 September 2026, with non-EU candidates ranked separately at their first-choice university. The circular also states that enrolment arrangements for international students on these courses will be set out in a subsequent note — so treat this as unsettled and re-check it before you plan a cycle.
Getting this wrong costs a year. Believe the headline, miss the IMAT registration window, and you wait for the next cycle.
The belief that is wrong
"Medicine abroad means an enormous tuition fee" is true of most destinations and false of Italy. In the countries Bangladeshi students usually go to for an MBBS, medicine is taught at private medical colleges, where the fee is priced the way a premium product is priced: against demand. We publish no figures for those countries, having not verified them. But hold on to the structure: there, an institution sets the price on its own commercial logic.
In Italy, a rule sets it. A public university applies the ISEE parificato to every degree it offers, medicine included — a government index built from your family's income and property, assembled from translated and legalised Bangladeshi documents. There is no separate medical fee. So "what does medicine cost" becomes two smaller questions: what is your family's assessed income, and did you file the document at all?

Year one: the money that actually leaves your hands
The Bangladesh half is spent before anyone decides anything, and none of it comes back.

That monthly EUR 450-800 breaks down as housing EUR 150-400, food EUR 120-220, transport EUR 25-45 and everything else EUR 60-130. The city moves it most: of those we cover, Milan is the most expensive, Bologna and Padua the most budget-friendly. Over six years that decision outweighs the tuition band.
Six years, added up
Medicine and surgery in Italy is a single-cycle degree — not a bachelor's then a master's, but one continuous course of six years. The cells below are our own arithmetic on the verified annual figures above, not a price any authority quotes.

Both ends buy the identical degree. The gap comes from filing an income assessment and choosing a city, not from a better or worse university. Costs ease from the second year, when deposits and one-off paperwork do not repeat (why), and the wider picture is in what studying in Italy really costs.
What the degree costs, and what you must show, are different numbers
This is where most confusion lives. For the visa you must evidence EUR 10,179.85 for one year. It is not a payment to Italy or a university fee, but proof that you can reach enough money to live for a year.

Four conditions travel with it. It is a single annual sum, and the circular publishes no monthly equivalent, so a monthly figure from any source was invented. Italy requires no blocked account, unlike Germany (the comparison). The funds must be lawful and traceable, accounts in the student's own name expressly preferred, failing that the family unit up to a relative of the fourth degree. And the consulate checks the origin against family income and assets, which is why a balance that appears suddenly reads badly (the full rule, and what an officer reads).
DSU: an income scholarship, not a merit one — and the trap inside it
DSU works no differently for a medical student. It is the regional right-to-study scheme, and it is income-based, not merit-based — a strong IMAT score does not win it; your family's income documents do. What it pays is not small: a tuition waiver, a dormitory place, subsidised meals and an annual grant. It runs region by region, each agency with its own deadline, separate from admission and often earlier, and eligibility runs through the same ISEE parificato (the detail, and which agency covers you).
Now the trap, which costs families a cycle every year: a scholarship you have only applied for is not money. Applying for a DSU award, or being eligible, cannot count toward the means of subsistence without an explicit attestation that it was awarded. So never plan the visa evidence around DSU. Treat it as what lowers your cost from the second year, and assemble EUR 10,179.85 independently. Same caution elsewhere — Invest Your Talent in Italy redefines its eligible-country list every edition, and Bangladesh has not been a consistent entry.
The costs nobody budgets for
Recognising your qualification. The university wants either a CIMEA statement of comparability or a Declaration of Value — alternatives, and the institution decides which. CIMEA is online, paid, and usually takes about a month. It is the slowest step in the Italian timeline, so start it first: Declaration of Value or CIMEA. Good news: Bangladesh joined the Apostille Convention on 30 March 2025 and Italy accepts an apostille, unlike Denmark, Greece and Finland, which objected (the map).
Language evidence. The institution must verify at least B2, and for a course taught in a foreign language the certificate must cover listening, reading, speaking and writing. There is no national "Italy requires IELTS 6.0" rule: 6.0 is the typical level we observe on programmes, a different claim. Health insurance. One kind of cover for the visa file, another after you arrive. Premiums are set by insurers, so we quote no figure: health insurance for a European student visa.
Getting there and the first month. A one-way ticket at BDT 70,000-120,000, plus a housing deposit, a first month's rent and the permesso di soggiorno kit. The list is in the first-year costs nobody budgets for, the journey in your first flight.
The cost of missing a date. The national visa deadline is 30 November 2026 for AY 2026/27 and 31 October 2027 for AY 2027/28, and as a single-cycle course medicine falls inside it; your university may set an earlier date (detail). The new 20 percent validation cap can stop a late but perfect file, because an institution may validate no more than its allocated international places plus 20 percent (explanation). Who should not take this route
Two kinds of reader should look elsewhere.
Anyone who will not clear the IMAT. The whole cheap-tuition argument rests on one condition: that you get in. The paper is 60 questions in 100 minutes, marked +1.5 correct, -0.4 wrong and 0 blank, for a maximum of 90. The cycle we last confirmed sat on 29 September 2026, registration open 26 August to 9 September 2026 at 15:00 Italian time; dates move every year, so read the current ones on Universitaly. If you will not give a year to serious preparation, Italy's low tuition is not an opportunity you have — it is a door you will not open. Dentistry is the wrong consolation, same test through a smaller door; the realistic move is public health, pharmacy and life sciences.
Anyone planning to work their way through. The ceiling is 20 hours a week, bankable across 52 weeks, capped hard at 1,040 hours a year under DPR 394/1999 article 14 — there is no full-time-in-the-holidays version (the rule). A medical timetable is the worst possible fit: lectures, labs and hospital rotations take the bulk of the day and cannot move around a shift. Part-time work closes a gap in a budget; it does not create one. A family that cannot fund the first two years unaided is taking a real risk: stopping halfway through medicine is the most expensive failure of all, and a partial medical degree converts into nothing.
One thing more, since it is often sold as part of the package: time on a study permit does not count toward Italy's five-year long-term residence route under TUI article 9 comma 3. After graduation there is a job-seeking permit of not less than 9 and not more than 12 months, where 12 is a ceiling, not an entitlement. Budget for a degree, not settlement.
What to do next
Decide first whether you will give a year to the IMAT; nothing else here is real until you do. Then start the CIMEA statement or the Declaration of Value today, the slowest step. In parallel, assemble the income and property documents for the ISEE parificato, because that one file sets six years of tuition. And build the EUR 10,179.85 separately, without assuming a DSU award. How much is tuition for medicine at an Italian public university?+
EUR 0-4,000 a year. The figure does not depend on the subject; it depends on your family's assessed income, through the ISEE parificato. A low band can bring it to roughly EUR 150-200 a year. Filing nothing puts you in the top band, commonly EUR 2,500-4,000.
What does the whole six-year degree cost?+
Tuition plus living costs works out at roughly EUR 5,600 to EUR 13,600 a year, so about EUR 33,000 to EUR 82,000 across six years. How much money must I show for the visa?+

Is the EUR 10,179.85 added on top of tuition?+
No. It is not a payment at all. It is evidence that you can draw on enough money to live for a year. The money stays in your account and you spend it yourself; nobody in Italy collects it.
If I apply for a DSU scholarship, does that count as my funds?+
No, and this is the mistake that sinks files. Merely applying for, or being eligible for, a regional right-to-study scholarship cannot count toward the means of subsistence. You need an explicit attestation that it was awarded. Plan the visa money as though DSU does not exist.
Can medicine in Italy be studied in English?+
At some universities, yes, through the IMAT. The language rule still applies: the institution must verify at least B2 CEFR, and for a course taught in a foreign language the certificate must cover all four skills. There is no national IELTS 6.0 rule.
What is the IMAT registration fee?+
The organisers set it and it changes between cycles, so we do not print a figure. Registration runs through Universitaly, where the current fee and the current dates are published together.
Can I work my way through a medical degree in Italy?+

No. Everything on this page describes public universities, where tuition is set by the income assessment. A private institution sets its own fee on its own commercial logic, and the income bands do not apply.
