Families in Bangladesh ask us this in almost the same words every year: can my son or daughter study medicine in Italy, in English, at a government university, for very little money. The answer is yes. That route genuinely exists, and on tuition alone it is the cheapest serious medical degree in Europe.
The reason it is cheap is the half of the sentence nobody says out loud. Italy does not ration those places by price. It rations them by one examination, sat once a year, against a separately counted pool of international seats whose size nobody publishes. This page is the mechanics of that examination, and who should attempt it.
What the IMAT is, and what it decides
The IMAT is the International Medical Admissions Test. It is the admission test for English-taught medicine and dentistry at Italy's public universities. It is not one input among several alongside your transcript and a personal statement. For these courses it is the thing that decides the outcome.

Read the last row twice. One sitting: a missed registration window is not an inconvenience, it is twelve months.
Private institutions set their own admission arrangements and their own fees. They are a different question, and none of the figures below describe them.
The calendar, and why we will not print next year's dates
Here is the last cycle we confirmed from the portal itself, checked on 14 September 2026.

That is the shape to plan against: registration in late August, a window of roughly two weeks, and the test in late September. We are deliberately not printing a 2027 date: the ministry publishes the cycle each year, and every page you find carrying confident future dates is guessing. Two things about that window catch people out. It closes at a fixed clock time in Italy, which in September runs four hours behind Dhaka, so a 15:00 Italian deadline is 19:00 in Bangladesh the same day. And it falls in the weeks when HSC results and admissions at home are at their noisiest, which is exactly when a family stops checking a foreign website.
The marking scheme is a strategy, not a footnote
One hundred minutes for sixty questions is one minute and forty seconds each, including reading. Speed under time pressure is a separate skill from knowing the biology, and the half most candidates never practise.
The marking matters more. A correct answer earns 1.5, a wrong one costs 0.4, a blank costs nothing. So leaving a question blank is a real, deliberate option priced at zero, and answering is a bet.
The arithmetic is worth doing once, properly, because almost nobody does. Answering pays when your chance of being right multiplied by 1.5 beats your chance of being wrong multiplied by 0.4, and those balance at a confidence of about 21 percent. Below that, guessing loses marks on average. Above it, it gains them.
In practice that is one rule: if you can confidently eliminate an option or two, answer; if the question is genuinely unfamiliar and you would be choosing at random, leave it. A candidate who knows this walks in with a scoring plan. One who does not either freezes and leaves a third of the paper blank, or fills in every bubble in the last four minutes and marks themselves down for it.
The order of operations for a non-EU applicant
This is where Bangladeshi applicants lose files. Two separate actions, both on Universitaly: you pre-enrol for a visa, and you register for the IMAT. Doing one is not doing the other.
Pre-enrolment is not a formality you complete after a result arrives. Since academic year 2026/2027 the circular has added a ceiling to it: once you submit a pre-enrolment application, the institution may validate no more than the number of places it has allocated to international students, plus a maximum of 20 percent, after which a monitoring mechanism activates. Without validation there is no visa appointment.
Read that carefully, because it is easy to overstate and we will not. The cap is on validations by the institution, not on visas, and the circular publishes no per-university allocations. What it changes is behaviour: pre-enrolment is no longer a queue that always has room, so a strong candidate who pre-enrols late can be refused validation because the allocation is exhausted rather than because the file was weak. One nuance cuts the other way: a refused visa returns that validation slot to the institution. The step-by-step is in Universitaly pre-enrolment.
The other hard date is the consulate's, and it binds this degree. The national deadline covers Italy's ordinary degree courses — the Laurea, the Laurea Magistrale and the single-cycle degrees — and medicine is single-cycle. For academic year 2026/2027 the application had to be lodged by 30 November 2026; for 2027/2028 it is 31 October 2027. A doctorate, an Erasmus placement, a single course or the Italian professional Master, which is not the Laurea Magistrale, sits outside that national date. Institutions may also set earlier deadlines of their own, tied to when teaching starts, so the ministry's date is the wall behind your real deadline. Plan to the university's.
How many seats there are, and why we will not tell you
Non-EU places for medicine are capped per university and counted separately from EU places. You are not competing with the whole applicant field. You are competing inside a small, separately allocated international pool at each university you name, and the size of that pool is set by that university for that year.
We publish no per-university numbers, because no central source publishes them and we will not repeat a figure we cannot open a page and verify. The consequence is practical: because those pools are small, the score that got somebody in last year tells you very little about this year. It is a weak prediction rather than a bar you can aim at.
The only reliable number is the one you assemble yourself, from the published call of each university you apply to, in the year you apply.
What it costs, and why that is the easy part
Italian public universities charge on assessed family income rather than a list price, which is why our dataset carries a band rather than a fee.

The six-year columns are arithmetic on a published band, not a quotation: your number depends on the assessment of your family's income and on the city. Set the top of that range against the roughly EUR 79,000 to 102,000 our MBBS in Bulgaria article puts on six years there. On price, Italy wins and it is not close. On admission, Bulgaria's written entrance examination in biology and chemistry is a far more predictable gate than a rank in a national test.
Two more things belong here. The EUR 10,179.85 is what the circular requires you to evidence for a year, raised from EUR 6,947.33 the year before, and it must come from lawful and traceable sources — your own money, or a family member up to the fourth degree of relationship, not a friend, an employer or an agency. Italy's regional DSU grants can lower the cost again, including tuition waivers and accommodation, through a separate application with its own deadlines — start it as early as the pre-enrolment.
The language rule is not IELTS 6.0
Italy's requirement for a course taught in a foreign language is B2 on the Common European Framework, covering all four skills — listening, reading, writing and speaking — evidenced by suitable certification, or by an equivalent certificate the university itself issues. IELTS is one accepted form of evidence. It is not the rule, and no national rule says 6.0. The routes are in studying in Italy without IELTS.
There is a second language fact that matters more for medicine than for any other subject, and it is the one applicants skip. Your lectures may be in English. Your patients will not be. Clinical years put you in Italian wards, taking histories from Italian-speaking people. Anyone who tells you that an English-taught medical degree in Italy means six years without Italian has either not done it or is selling you something. Start on Italian in the year you sit the test, not in the year you reach the wards.
Who should not sit this test
This is the most important paragraph on the page, so we will be direct.
If your plan rests on a strong HSC result, this is not your route. The IMAT does not read your transcript. A good HSC in biology and chemistry is what makes you eligible to sit, and Italy separately requires a final secondary qualification representing at least twelve years of schooling. Eligibility is not a score. Admission is a rank on one paper against a pool competing for a small, separately counted allocation, and most of the people in that pool also had strong school results.
If you cannot afford the year, do not build the plan on this. One sitting, once a year, and no second chance inside the cycle. Families who treat the IMAT as the plan rather than one branch of it lose a year, then decide worse in a hurry.
If somebody has quoted you a success rate, stop and check where it came from. Nobody publishes an admission rate for Bangladeshi IMAT candidates, and nobody publishes an Italy student-visa approval rate for Bangladesh either — type D national visas do not appear in the Schengen statistics, which cover short-stay visas only. If you want to practise in Bangladesh, understand the last step before the first one. A foreign medical degree does not by itself let you practise at home. Registration with the Bangladesh Medical and Dental Council is a separate process, normally including an internship and the registration qualifying examination, and it applies wherever you studied. Confirm that the university is on its recognised list before you pay a deposit, not after six years.
If the answer is no, these are the honest alternatives
Being unsuited to the IMAT is not the same as being unsuited to medicine, still less to healthcare.
- A more predictable medical gate. Bulgaria and Hungary admit on a written entrance examination in biology and chemistry rather than on a rank. It costs several times as much, and it behaves like an exam you can prepare for rather than a competition whose bar moves each year.
- Dentistry, which runs through the same IMAT but is a different decision. Studying dentistry in Italy in English.
- The health degrees that need no clinical licence — public health, biomedical science, pharmaceutical science, nursing science — at a fraction of the cost and with none of the language wall. Start at the health routes that are not medicine.
What to do next, in order
- 1Open Universitaly and read the current IMAT notice. Put the open date, the close date with its Italian clock time, and the test date on a calendar your parents can also see.
- 2Sit a full past paper under a timer, at sixty questions in one hundred minutes, before you decide anything. That single afternoon tells you more than any counsellor will.
- 3Decide your blank-answer policy now, using the 21 percent rule above, and practise with it rather than discovering it on the day.
- 4Read the whole visa sequence once, end to end, before September, so the pre-enrolment step and its ceiling do not surprise you in the middle of the test window.
- 5Build the same year with a second country in it. Not as pessimism — as arithmetic, given one sitting a year.

Common questions
Can I study medicine in Italy in English?+
Yes. Several Italian public universities run the six-year single-cycle medical degree in English, and admission to those courses is decided by the IMAT, the International Medical Admissions Test. You register for it on Universitaly, the same ministry portal used for visa pre-enrolment. Private institutions set their own admission arrangements separately.
How is the IMAT scored?+
Sixty questions in one hundred minutes. A correct answer scores plus 1.5, a wrong answer minus 0.4, and a blank scores zero, so the maximum is 90. Because a wrong answer carries a penalty, answering is only worth it when you are more than roughly 21 percent confident, which in practice means answering whenever you can eliminate an option and leaving genuinely unfamiliar questions blank.
When is the IMAT held?+
Once a year, with registration in late August and the test in late September. The last cycle we confirmed was the 2026 sitting on 29 September 2026, with registration open from 26 August to 9 September 2026 at 15:00 Italian time. How much does medicine in Italy cost for a Bangladeshi student?+
At a public university tuition is assessed on family income rather than charged as a list price, and our dataset carries a band of roughly EUR 0 to 4,000 a year. Living costs run about EUR 450 to 800 a month. Separately, the visa requires you to evidence EUR 10,179.85 in available funds for the year, and the type D study visa fee is EUR 50.
How many places are there for non-EU medical students in Italy?+

The practical effect is that you compete inside a small pool at each university, which also means a previous year's cut-off score is a weak guide. Do I need Italian if my medical course is taught in English?+
Yes, eventually and unavoidably. Admission requires B2 in the language of instruction across all four skills, which for an English-taught course means English. But clinical training happens in Italian hospitals with Italian-speaking patients, so you will need working Italian during the degree. Treat Italian as part of the course rather than as something optional.
